Medical Marijuana Card in New Mexico
To assist patients in the enrollment process to apply for a NM Medical Cannabis card:
New patient without insurance=$100.00
New patient with insurance=$50.00
Renewal without insurance= $40
Renewal with insurance=$25.00
Caregiver application=$30

Patients in New Mexico diagnosed with one of the following severe, debilitating, or life-threatening medical conditions, are afforded legal protection under the New Mexico “Lynn and Erin Compassionate Use Act” and qualify for medical marijuana:

Please note:
- If applying under Chronic Pain, the patient MUST have a second certification from a pain specialist or 2 previous physician notes dated in the past year that stated your diagnosis and symptoms. If applying under PTSD, the patient MUST have medical records showing a psychiatrist’s diagnosis.
- If applying under Glaucoma, the patient MUST have medical records showing an ophthalmologist’s diagnosis.
- If applying under Chronic Inflammatory Demyelinating Polyneuropathy-the patient MUST have medical records from a Neurologist showing the diagnosis and symptoms.

If applying under Crohn’s Disease -the patient MUST have a second certification/medical records from a Gastroenterologist showing the diagnosis and symptoms.
How to apply for the medical Marijuana card:

Patient must be eighteen (18) years of age. If patient is under eighteen (18) years of age, patient must designate a Caregiver.

Patient must provide copy of New Mexico State ID.
-
Patient must be evaluated, in person, by a licensed medical provider.
-
Patient must be diagnosed with a Qualifying Condition.

Patient must submit application to New Mexico Department of Health Medical Cannabis Program.

There is no charge to apply. There is a $50.00 fee for replacement card.
